HOW DO I SCHEDULE A SESSION?

 

To get started, please fill out the "Appointments & Inquiries" form on the Contact page.  Be sure to include the reason for your nutrition consult and you will receive a response within 1 to 2 business days about setting up your first session.

 

DO YOU ACCEPT INSURANCE?

 

At this time, I do not accept payment through private insurance coverage, Medicare, or Medicaid.  However, I can provide you with a superbill upon request if you would like to submit a healthcare claim for reimbursement on your own, but there is no guarantee they will approve it. It is your responsibility to check with your insurance provider prior to your appointment to determine if nutrition counseling is covered by your plan.

 

I can also provide you with a physician referral form that may help when submitting for insurance reimbursement.  Email me and I will send you the form in order to request a referral from your physician. Please bring the completed form to your Initial Consult.

 

WHERE DO NUTRITION CONSULTS TAKE PLACE?

 

Sessions are conducted in-person at my Hyde Park office OR virtually via video chat, or phone. 

 

WHAT IF I LIVE OUTSIDE OF THE CINCINNATI AREA?

 

I would still love to help you with your goals! Nutrition Coaching services are also available virtually via video chat, or phone. 

 

WHAT FORMS OF PAYMENT DO YOU ACCEPT?

 

Methods of payment include; cash, check, credit, or debit. All payments are due at the time of your scheduled consultation.
